How Can severe Back Pain Be Treated?

Hi, My husband has Type 2 diabetes and takes medicine for high blood pressure and high triglecerides also. My husband has had severe back pain off and on for about 1 week now. He has dull pain in his thighs and had chills last night and says his foot feels like it is frostbit. He could not sleep most of the night and said when he urinates it stings. He refuses to see a doctor. Thanks
